TIP! Before starting the form, be ready to provide your date of birth, Crew ID, passport number, registered email address in C-TRAC, and Seaman’s book (if applicable). Also, you cannot save your entry and return to the form later. All entries will be lost, so make sure you make enough time to fill out the form completely.

Step 2: Make an appointment with one of our approved medical providers to get your physical examination performed.
Step 3: Once you have received your Medical Certificate for Service at Sea, please upload it to your C-TRAC account.
